Getting Enemeez Covered by Medicaid in Approved

Obtain a prescription from your
Doctor. |

Take to your Medicaid Approved
Pharmacy (Where you are currently receiving your prescriptions
through Medicaid) |

If you receive a denial in a Medicaid
approved State listed below, ask the pharmacy the reason for
the denial and

Jean will work with your pharmacy in
getting the submission approved. |
For Residents of California who are on Medi-Cal:
Once the Pharmacy receives your prescription, it will be denied.
To get an approval, you must request a TAR (authorization request)
from your pharmacy who will work with your physician to complete
and submit.
*Please note: Some states require a prior authorization and your
pharmacy and your physician will take care of this for you.
**States are required by law to respond to any TAR or Prior
Authorization they receive within 48 hours. |
